Carmen founded Sample Supports in 2010 with the belief that every individual deserves a place in the community. As a social entrepreneur, clinician, and former host home provider, Carmen is passionate about building inclusive spaces for individuals with intellectual and developmental disabilities. Under her leadership, Sample Supports has grown into a comprehensive agency offering residential services, day programs, employment services, and more — all centered on high-quality, person-centered care. Outside of work, Carmen enjoys spending time with her husband and four children, traveling, exploring the mountains, and engaging in the vibrant Longmont community. She looks forward to connecting with you!



​Alexa Cataldo, LCSW
CEO

Alexa has been an integral part of Sample Supports for over a decade, currently serving as CEO. With experience spanning direct care, clinical services, and operations, she brings a deep understanding of all facets of the organization. Alexa earned her Master of Social Work from the University of Denver in 2017 and became a Licensed Clinical Social Worker in 2019. She is passionate about community-based, person-centered care and advocates strongly for deinstitutionalization, believing that every individual, no matter how complex their needs, deserves to thrive within their community.





Bri Kurt-Hurst, MA, LPC, NCC, ADDC
Boulder County Regional Director
On the Board of Childsafe Colorado

Bri serves as the Boulder County Regional Director, bringing a strong background in clinical mental health counseling and disability services. She holds a Master’s degree from the University of Northern Colorado and is a Licensed Professional Counselor, National Certified Counselor, and Licensed Addictions Counselor Candidate. Since joining Sample Supports, Bri has contributed in various roles including Direct Service Professional, Supported Employment Team Lead, and Regional Director. Bri specializes in supporting individuals with disabilities, trauma histories, and dual diagnoses, and is deeply committed to creating pathways for recovery and empowerment.






Sara Burnett, LCSW
Northern Colorado Regional Director

Sara has been part of the Sample Supports team since 2016, beginning her journey as a Direct Support Professional before advancing into leadership roles within the residential program. She holds a Master of Social Work from Colorado State University and is a Licensed Clinical Social Worker, with additional training in EMDR therapy. As Northern Colorado Regional Director, Sara is committed to the belief that no individual is "too hard" to serve. She values the agency’s commitment to providing unwavering support to both individuals and employees, creating a strong foundation for community inclusion and growth.

Alyse Sumanis, MSW
Denver Regional Director

Alyse serves as the Denver Regional Director, bringing over 13 years of experience in disability services and behavioral health. She has grown within the Sample Supports team, having previously held roles as a Direct Support Professional, Behavior Specialist, and Behavior & SLS Program Manager. Alyse earned her Master of Social Work from Colorado State University and is currently pursuing her clinical licensure. Outside of her professional role, Alyse enjoys hiking, camping, snowboarding, and spending quality time with her family, friends, young son, and beloved dog, Wall-E.
