Adult Transition Services to HCBS Waivers

Sample Supports provides guidance throughout the process of transitioning individuals with intellectual and developmental disabilities from youth to adult services through HCBS waivers. 

The Sample Supports Way:

• Communication with the individuals' local Case Management Agency to ensure no lapse in support
• Advocacy to secure appropriate long-term funding based on an individuals' needs
• Assistance in understanding differences between Supported Living Service and Comprehensive Service Waivers
• Assistance in choosing the correct waiver, based on an individuals' need
• Guidance throughout the process of applying for and receiving Social Security Income 
• Support around identifying a legal guardian and completing that process, if necessary
• Identification of long-term adult residential supports, if needed. Youth can remain in the same foster home into adult services through Sample Supports!
• Seamless transition of comprehensive service plan from child welfare to adult I/DD services
